COMPANY POLICY TERMS AND CONDITIONS
APPOINTMENTS: All massage, reiki, life coaching, tarot and life coaching
Appointments are by appointment only. Massage appointments require a $30 deposit fee to hold the appointment and is later applied to the total cost of services. Gift certificates may also be used in lieu of payment to hold your appointment.
ARRIVAL TO YOUR APPOINTMENT: New first time clients for body work appointments are asked to arrive 10-15 minutes early. This allows time to fill out the appropriate client forms. Established clients should arrive 5 minutes before their scheduled time. All messages have a specific time schedule and early arrival allows for a relaxed and unhurried experience.
LATE ARRIVAL POLICY: We regret that late arrivals will not receive extension of scheduled appointments. In special cases, and when our schedule will allow, we may be able to accommodate a partial or full appointment. This will be at our discretion and only with proper, advanced notification of your late arrival. If late arrival is inevitable, your service(s) may be shortened in order to keep on schedule. The original treatment time will be charged. With inclement weather, the office will open unless it is a Level 2 or 3 emergency.
NO-SHOW POLICY: Clients who do not show without canceling per policy standards are still responsible for payment and will be charged the $30 cancellation fee and full travel costs if this was an out of the office visit.
CANCELLATION POLICY: Unforeseen events occur requiring cancellation of your appointment. Since the massage therapists work by appointment including Life coach, they do not receive compensation for the appointments that are missed. In our commitment to provide a unique and outstanding massage experience to all of our client and out of consideration of our therapists’ time, we have adopted the following policies:
Please provide at least 24 hours notice if you need to reschedule or cancel a treatment. This gives us enough time to fill the slot. We make our reminder calls/email/text also 24 hours prior to your scheduled appointment in effort to confirm your appointment. This is to benefit you and your deposit. You are welcome to make changes at that time, so please pay attention to voicemails, emails, or texts from us. We work weekends so the notice does not apply to business days.
Appointments canceled at the time of the appointment or the day of the scheduled appointment time will be charged the cancellation fee of $30 making the deposit NON-REFUNDABLE.
In the event you used a Gift Certificate to reserve your appointment but failed to reschedule your appointment, or show for your appointment without canceling prior, the gift certificate is then void. Please be sure to call and reschedule accordingly.

We recognize the time of our clients and staff is valuable and have implemented this policy for this reason. When you miss an appointment with us, our therapists not only lose your business, but also the potential business of other clients who could have scheduled an appointment for that same time. Additionally, many times our staff will be specifically for your service. For these reasons, we are obligated to compensate our staff for their time.
*If you choose to reschedule your appointment before/at the 24 hour notice your original deposit fee still stands as a deposit and will be applied to your next appointment. It may be refunded if you choose not to reschedule.
TRAVEL EXPENSE: In-home massage/Reiki sessions are subjected to a $15 setup/tear down fee and all services outside of the office are charged $1 for every mile traveled.

INCLEMENT WEATHER POLICY: If an onsite service is canceled by the client for inclement weather, no fee will be charged to the client as long as the event is rescheduled for a future date and meets the National Weather Service definition of a “Dangerous Weather Event”. To qualify for having the fee waived, NOAA & The National Weather Service should have issued a Major Storm Warning either during the day of the event or in advance of an expected event. (ex. The expected hurricane/tornado path). You can check warnings at https://www.weather.gov/.
The therapist has the right to cancel an event in the case of inclement weather if it jeopardizes the safety of our practitioners.
PAYMENT: Massage services require a $30 deposit fee that will be applied to your services. If the cancellation policy has not been met, the 24 hour notice of cancellation, or no show of a client, the $30 deposit fee now becomes a $30 cancellation fee. We accept payment through Venmo, Cash, and card if a card reader is available.
TREATING MINORS IN MASSAGE: The therapists are trained and prepared to work with children. If the child is younger than 18 years old, a parent or guardian has to be present upon arrival and sit in on the session. The parent or guardian must file out a consent form for the minor before the session can begin. For treatment of the minor, all undergarments must stay on. The massage protocol for minors is as follows:
Head, neck, shoulders, arms, back, legs, hands, feet. What is excluded from a full body massage is the inner thigh, glutes, chest (pectoralis treatment or anything 2 inches below the collar bone), and abdominal work.

PRIVACY POLICY: Your massage therapy and life coaching records are kept in the strictest confidence by this company. All client records are kept in a secure place and only those who need to see a client's file for legitimate business or professional purposes have access to them. Your records will not be released to third parties, including health care providers and insurance companies without written consent. Records may be surrendered by law.
CONDUCT: Inappropriate behavior will not be tolerated. Suggestive comments, sexual innuendos, touching of the massage therapist, life coach, practitioner in an inappropriate way, will result in the termination of the session and the client will not be allowed to return for further services. Our FULL FEE OF THE ENTIRE SERVICE will be expected.

LOTIONS AND CREAMS: Lotions and perfumes are not recommended prior to massage therapy as they can interfere with the lotions, creams, or oils used for the massage treatment session. If you have an allergy to any lotion, cream, oil, etc. or if you require a special product please let us know BEFORE your session. You are welcome to bring with you a preferred lotion, cream, or oil.
SICKNESS POLICY: We recognize that massage therapists, coaches, practitioners and clients are vulnerable to infections and therefore ask clients to cancel appointments when they are feeling unwell. If you have any covid symptoms or exposure within the last 5 days of your appointment please call and reschedule your appointment. The scheduling protocol is you must be symptom free for 48 hours before your appointment.
We cannot treat you if you have the following:
Diarrhea, Vomiting, Fever, Chicken Pox, Measles Mumps, Hepatitis A, Conjunctivitis/Pink eye, Rubella, Head lice, Impetigo, Influenza, Meningococcal Disease, Polio, Ringworm of the body/hands/feet, Scabies, Thrush, Whooping cough, The common cold, Covid, Flue, Monkeypox, Herpes Simplex outbreak HSV2 (or you believe you have viral shedding Symptoms).

In the event our Therapists, practitioners, or coaches contact you to cancel your appointment due to sickness, your $30 deposit is fully refundable or you may reschedule using your existing deposit.
SCOPE OF PRACTICE: Our licensed Massage Therapists are professionals and are held to the highest of standards of the American Massage Therapy Association. Massage Therapy is a profession in which the practitioner applies manual techniques, and may apply adjunctive therapies, with the intention of positively affecting the health and well being of the client. Licensed Massage Therapists do not diagnose or prescribe for medical conditions. Our services are not intended as an alternative for proper medical attention for specific conditions. Please refer to a medical doctor for any specific condition which requires medical treatment. Life Coaching is not an alternative to professional mental health services. It is not therapy or counseling. It is however an excellent addition to your mental health professional services.
EXISTING AND NEW MEDICAL CONDITIONS: It is the responsibility of the client to keep the massage therapist informed of any medical treatment currently being taken, and to provide written permission from the physician, chiropractor, physical therapist, etc., that the massage may be continued. The client must also keep the massage therapist informed of any changes in health conditions.
For clients undergoing chemo and radiation therapies - please note that we require a doctor's note that states the doctor is aware of and agrees to the desired treatment.

ALCOHOL/DRUGS DO NOT MIX: Massage Therapist and client both will refrain from alcohol and/or drug use for at least 12 hours prior to the massage session. If you are taking or have taken prescription drugs for pain management or anti-inflammatory purposes in the past 12 hours, please notify the massage therapist prior to the session beginning for rescheduling. If you are under the influence you will be refused service and charged fully for the complete service. We have a ZERO TOLERANCE for clients under the influence and it is contradicted.
*If you are in use of narcotic drugs due to injury please let the therapist know to assess if the massage is contraindicated or not.

HYGIENE: Massage therapy involves the use of touch, and may include the use of oils, lotions, or creams. Coming to your massage therapy session with a clean body is imperative for the health and safety of both the client and massage therapist. Personal hygiene is mutually respected by both the client and the massage therapist. If the client cannot uphold their personal hygiene, the client may be charged $30 for the session and the session will be postponed or rescheduled. If the client cannot maintain personal hygiene a 2nd time they may not be welcomed back for any services.
INFORMED CONSENT: Prior to each massage session, the treatment plan will be discussed with you. At your first visit with us you will be asked to sign the treatment consent form stating that you have read the information, understand it, and agree to comply with the professional massage therapy policies and procedures. Clients who we have not seen for at least a year may also be asked to fill out this form.
*Please notify the receptionist of any allergies or sensitivities to aromas or ingredients when you schedule your appointment.
RESPECT FOR CLIENT NEEDS AND BOUNDARIES: Our Massage Therapists are happy to adjust pressure, temperature, musical volume, work longer on area or move on if you request it. The client may choose to: leave on as much clothing as needed for comfort, refuse any massage methods, stop massage at any time. The client will always be modestly draped. Only the area being massaged will be un-draped. The clients will be kept informed of the area being massaged.
Sexual interaction or discussion of any kind between the client and the massage therapist is NEVER appropriate.
CONFIDENTIALITY: We treat all client conditions as confidential. You may choose to talk or not talk during your massage and it is confidential. Conversation will be guided by the client's direction. ALL Life Coaching information discussed is confidential. ALL Tarot reading findings are confidential.
Only time confidentiality is broken is at the discretion of the practitioner due to suspicion of SEX TRAFFICKING, HUMAN TRAFFICKING, or CHILD ABUSE. They will be reported to the police.

RELEASE: By booking an appointment and completing our informed consent form, the client affirms that they have stated all known medical conditions and agree to keep the therapist updated on their medical profile. The client also agrees that there shall be no liability on the therapist’s part, or on the part of LIFE PATH CONNECTIONS & MASSAGE THERAPY, LLC, if the client fails to do so. The client also agrees that a physician's note will be necessary before the beginning massage therapy treatment if they present with high risk complications.
